According to a recent consensus article in the Journal of Orthopaedic Research, topical nonsteroidal anti-inflammatory drugs (NSAIDs) are recommended as a first-line therapy for pain associated with osteoarthritis (OA). The use of topical NSAIDs is supported by clinical practice guidelines from the American Academy of Orthopedic Surgeons and the American College of Rheumatology. Diclofenac gel is the most commonly used topical NSAID, but other NSAIDs in various forms such as creams, foams, sprays, ointments, plasters, and patches have also been used. However, the effectiveness of topical NSAIDs can be diminished due to mechanical removal by movement, washing, and abrasion. To address this issue, adhesive bandage strips with diclofenac gel can be used to prolong the retention of the NSAID on the skin and prevent migration of the drug. This technique can be particularly useful for active OA joints such as the knee or small joints in the hand. The authors suggest that further research and experience with occlusive dressings and patches for extending the use of topical NSAIDs would be valuable. [Extracted from the article] |
