Over the last two decades, the United States has experienced a severe opioid epidemic, and is now facing its most deadly one. The latest surge in overdose and deaths is due in part to users moving from prescription opioids and heroin to fentanyl. There have been four times as many drug-related deaths or 307.6% more incidents when compared to homicidal deaths in January of 2021 (NCDAS, 2022). Addiction is not a character flaw or moral weakness, but a chronic disease that has been treated effectively by Medication for Opioid Use Disorder (MOUD). Yet, the stigma that surrounds individuals with opioid use disorder (OUD), harm reduction techniques and treatment is persistent; there is no logic behind this stigma. This paper reviews commonly used harm reduction techniques found to be effective: Needle Exchange Programs; Safe Injection facilities; MOUD; Smartphones; Certified Peer Supporters; and Mindfulness-based prevention. The paper provides evidence for the clinical effectiveness of MOUD: Naloxone; Buprenorphine (Suboxone); Methadone; and Naltrexone (Vivitrol). The challenges in breaking down the stigma surrounding OUD, harm reduction and MOUD are discussed, as well as presenting a holistic model for OUD treatment and recent policy changes and future policy suggestions.
