Abstrakt: This article discusses the management of a case of bilateral fourth cranial nerve palsy in a 30-year-old man who experienced head trauma in a motor vehicle accident. The patient presents with double vision, tilted diplopic images, and horizontal diplopia. Various treatment options are discussed, including inferior oblique weakening procedures, superior oblique tuck or Harada-Ito procedures, and asymmetric superior oblique full tendon advancements. The importance of conducting a thorough examination, including MRI scans, to determine the underlying cause of the palsy is emphasized. The article also highlights the effectiveness of superior oblique surgeries in cases of palsy, despite the muscle being completely palsied. [Extracted from the article]
