A recent report discusses research conducted at Louisiana State University on the technique of circuit exchange in high-risk patients on extracorporeal life support (ECLS). The traditional method of replacing the entire circuit at once can lead to hemodynamic deterioration in patients fully dependent on ECLS. The researchers propose a staged approach using a parallel circuit configuration to reduce the risk of physiologic decompensation. This approach provides hemodynamic and physiologic stability even in patients with absent underlying cardiopulmonary function.
