A 58-year-old man presented at the Moorfields eye hospital emergency department with swelling and discharge in his right cheek and eyelids. He reported pain in his teeth in the right upper jaw but no history of sinus-related disease or animal bite. On examination, swelling and redness were observed, and a small hole with discharge was found on the upper cheek. Computerized tomography scanning revealed extensive swelling in the malar, facial, and infraorbital tissues on the right side, as well as an osseous erosion near the right upper second molar tooth. The patient was diagnosed with orbital cellulitis related to dacryocystitis and underwent surgical drainage and antibiotic treatment. He showed significant improvement within one week. The article emphasizes the importance of early treatment to prevent vision deterioration and highlights the need for a multidisciplinary team evaluation and imaging for accurate diagnosis.
