This document is a reply to Li and colleagues regarding a systematic review and meta-analysis on the association between lean non-alcoholic fatty liver disease (NAFLD) and the risk of incident gastrointestinal (GI) cancers. The authors acknowledge the limitations of their study, including the lack of additional data for the included observational studies and the exclusion of liver-death outcomes. They agree with Li and colleagues that the statistical adjustments in the eligible studies are questionable and that liver fibrosis should be considered as a confounding factor. The authors also clarify that their study does not support prioritizing cancer screening strategies for lean NAFLD patients, but rather suggests investigating the feasibility and potential reduction in cancer-related mortality of these strategies.
