This article discusses the findings of a study on health promotion, specifically focusing on health-related social needs (HRSNs) such as food insecurity and housing instability. The study highlights the increased prevalence of HRSNs due to the socioeconomic impacts of the COVID-19 pandemic and emphasizes the need for strategies to address these needs, particularly in communities experiencing health disparities. The research synthesizes promising implementation approaches and lessons learned from the Accountable Health Communities Model, which aims to systematically screen for and address HRSNs in clinical settings. Key strategies include patient engagement, culturally tailored language and outreach, data utilization, technology integration, dedicated staff roles, workflow integration, and staff buy-in.
