A recent report from the Centers for Disease Control and Prevention (CDC) discusses three cases of prolonged mpox disease in individuals with advanced HIV who were not receiving antiretroviral therapy (ART) at the time of diagnosis. The researchers used various medical countermeasures, including prolonged tecovirimat treatment and immune optimization with ART initiation. Skin biopsies showed a dense immune response and abundant replication of monkeypox virus (MPXV) in epithelial cells and dendritic cells. These findings suggest that ongoing MPXV replication may contribute to prolonged mpox in individuals with advanced HIV, highlighting the need for aggressive medical interventions and immune optimization.
