Eleven cases with delusions of parasitosis are described. Eight are regarded as examples of a single clinical entity, the favoured diagnosis being a monosymptomatic hypochondriasis arising in an obsessional personality. The patients' reaction to this belief depends on their previous personality, the energetic obsessional being the most strenuous excoriator and fumigator, while less driving personalities react with a more placid picture of anxiety and depression.
