Abstrakt: A study conducted by researchers at the University of Cologne aimed to identify subtypes of patients with isolated REM sleep behavior disorder (iRBD), which is considered a specific marker of prodromal Parkinson's disease (PD). The study involved comprehensive clinical assessments of 66 patients with iRBD, and a two-step cluster analysis revealed two distinct subtypes based on various factors such as olfactory function, electroencephalography frequency, depressive symptoms, and disease duration. One subtype was characterized by a higher non-motor symptom burden and widespread reduction of cortical grey matter volume compared to the other subtype. The findings suggest the existence of clinical subtypes in the prodromal stage of PD and highlight the need for further longitudinal studies to investigate the risk of more aggressive phenotypes and dementia development.
