Adherence to interventions, a decisive factor for the effectiveness of treatments for smoking cessation, is still a challenge. The objective of this study was to evaluate adherence to computational technologies for the treatment of smoking compared with adherence to drug therapy, behavioral therapy, or computational technology different from that of the intervention group. This is a systematic review of clinical trials and meta-analyses in which MEDLINE, Cochrane Library, Scopus, Web of Science, and Lilacs were consulted, with the last search conducted on September 5, 2020. Two reviewers performed study selection, quality assessment, and data extraction with disagreements resolved by a third reviewer. The review included 13 studies, most with a follow-up of fewer than 5 months and published between 2014 and 2018. Compared with that in the control group, the adherence to computational technology in the intervention group was higher (RR = 1.19; 95% CI = 1.03–1.37). The combination of technology and nicotine replacement resulted in greater adherence (RR = 1.18; 95%CI = 1.11–1.25) than did noncombination (RR = 0.90; 95%CI = 0.90–1.03).
