This article discusses a case report of a 54-year-old female with chronic actinic dermatitis (CAD) and fragrance sensitization. CAD is a condition characterized by eczematous lesions in sun-exposed areas and a decreased minimal erythema dose. The cause of CAD is unknown, but it is believed to be an immunologically mediated response to either exogenous or endogenous skin antigens. The patient in the case study had a history of chronic hand eczema and tested positive for various allergens. Despite treatment, her condition worsened over time, leading to lichenified plaques and generalized itching. Phototherapy was ineffective, and alternative diagnoses were ruled out. The patient eventually started treatment with dupilumab, a medication that showed promising results in the treatment of CAD. After starting dupilumab, the patient experienced progressive improvement, with a decrease in lesions and pruritus. The article suggests that dupilumab may improve the Th2-mediated allergic contact dermatitis present in many CAD patients. Overall, the case study highlights the diagnostic and therapeutic challenges of CAD and proposes dupilumab as an optimal treatment option. [Extracted from the article] |
