Abstrakt: The most significant feature of Besnier's prurigo is an abnormality of immunologico -autonomic mechanisms. The incidence of abnormal electroencephalographic tracings is significantly higher in Besnier's prurigo than in the general population. Adrenaline, liberated by emotional and other stimuli but not dispersed in action in the vagotonic individual, may be responsible for the periodic liberation of histamine and general pruritus. Itching may be widespread, and the localization of the dermatosis to the face, neck, flexures, wrists and elsewhere is determined, in part, by the easier accessibility of these areas ; in part by the higher content of histamine in the skin of the face and neck ; in part, once the lesions have become established, by the greater influence of circulating histamine on areas of skin partially depleted of histamine than on areas not so depleted. In white dermographism the cells of the skin at the affected sites appear to have become refractory with the result that they do not release histamine in response to frictional stimuli. In this skin disease, which is often accompanied by ichthyosis and sometimes by cataracts and in which abnormal electroencephalographic tracings are often found, it is probable that the fundamental pathological process pervades the whole epiblast (ectoderm).
