* BACKGROUND: A 28-year-old recreationally active female sustained a lateral meniscus injury that was managed by undergoing a lateral meniscectomy and debridement. * CASE PRESENTATION: Postoperative recovery was complicated by a hemarthrosis (joint bleed) related to a rare bleeding disorder called Factor XI deficiency or hemophilia C. * OUTCOME: This case report outlined the timeline of events including presurgical planning, initial outpatient physical therapist visit, hemophilia treatment center hemarthrosis management, return to outpatient physical therapist, and progression to running. * DISCUSSION: Rehabilitation of younger, active patients looking to return to impact sports, such as running, is not well published in patients with a history of joint bleeds.
