The article presents case study which illustrates acute osteomyelitis of patella and records an association between convulsions and intravenous gentamicin therapy. Primary involvement of the patella is an uncommon form of acute osteomyelitis and Pseudomonas aeruginosa is a rare etiologic agent in any form of acute osteomyelitis. Physical examination revealed limitation of joint motion, inability to bear weight, and edema over and around the left patella. The circumference of the left knee was 33 cm; of the right, 29 cm. A hemogram and urinalysis were normal, but the sedimentation rate was 28 mm. Roentgenologic examination of the knee revealed marked soft tissue enlargement, increase in joint fluid and no bony changes. Culture of five ml of aspirated cloudy joint fluid was negative for bacteria. Tomograms of the left patella were normal on the eighth hospital day, but repeat study on the 15th day showed a 1 cm linear density of the inferior patellar margin. The knee was surgically explored with removal of 20 ml of pus, and a sinus tract to the medial aspect was discovered. |
