Toxic Digitalis Levels Associated with Indomethacin Therapy in a Neonate.

Abstrakt: The article details the case of an 1,800-gram male infant born after a 33-week gestation period developed ascultatory findings of patent ductus arteriosus (PDA), plus bounding peripheral pulses, at age 11 days; 9 days after resolving his mild symptoms of respiratory distress syndrome. Chest radiographs revealed cardiomegaly and pulmonary congestion. The article reports an instance of toxic serum levels associated with the interaction of digitalis and indomethacin therapy in a premature infant with a PDA. This case history suggests that an elevation of the serum digitalis level to the toxic range and prolongation of the serum half-life may occur when combining digitalis therapy with indomethacin. While such an increase in an infant suffering from congestive heart failure may also result from the reduced glomerular filtration rate secondary to the heart failure itself, the simultaneous administration of indomethacin, a known depressant of renal function, potentiates the risk of toxicity.
