Abstrakt:	The article aims on study focuses on implementing a pilot VIONE PharmD Telephone Clinic within an Older Adult Patient-Aligned Care Team at the West Palm Beach VA Healthcare System to optimize medication regimens for patients receiving palliative-focused care. The study found that clinical pharmacist practitioners using the VIONE model successfully deprescribed medications, with an average of two medications per patient, resulting in potential cost savings and minimal harm to patients.
