Patient participation during discharge medication counselling: Observing real-life communication between healthcare professionals and patients.

Abstrakt: This qualitative study in the Netherlands evaluated the communication between healthcare professionals (nurses and pharmacy technicians) and patients during discharge medication counselling. Thirty-four discharge consultations were observed, audio recorded and analysed. The healthcare professionals used cues to increase patient participation. For example, they asked about patient's preferences, showed empathy and support, and verified that the information had been understood clearly. They took a leading role in the counselling process to ensure the transmission of information to patients. Some patients participated in discharge medication counselling, by asking questions and expressing concerns. This was influenced by the timing of discharge consults, the individual healthcare professional, and the presence of a relative. [ABSTRACT FROM AUTHOR]
