In peripheral arterial occlusive disease ideal conditions may be present in poorly perfused tissue areas for the development of anaerobic infections. Particularly ischaemic ulcers and gangrene of the lower extremity may offer appropriate conditions for entry and multiplication of tetanus spores, followed by toxigenesis. Of the mostly older patients at risk a large number have no documented immunisation or adequate antitoxin levels. On the basis of our experience with one lethal tetanus infection following femoro-popliteal revascularisation we now recommend that tetanusimmunisation be a part of the therapeutic regimen in all patients with peripheral arterial occlusive disease.
