The transvenous system of permanent pacemaking is both simple and safe and is recommended, especially for older patients. The standby (demand) pacemaker functions in all types of symptomatic bradycardia, virtually eliminating the danger of competitive pacing, and is, therefore, recommended in preference to the asynchronous (fixed rate) pacemaker. The continuously functioning synchronous standby R.S. pacemaker is currently preferred to the intermittently functioning R.I. type, because of the possibility of pacer inhibition by stray electrical fields in the latter. With better protection against external electrical fields by improved circuit design, the R.I. type of demand pacemaker will be the preferred unit because it does not distort the QRS complex and should have longer battery life.
