Abstrakt: This article focuses on the acute fragility of cerebral blood vessels in hepatic coma. Early in hepatic encephalopathy the patient may show mild tremulousness and restlessness. There may be a reversal of the normal sleep pattern. Errors in judgement and forgetfulness follow. Mental sluggishness, inability to concentrate and lethargy become evident. Increased deep tendon reflexes, tremor, spasticity and even convulsions may be part of the clinical picture before coma supervenes. One of the features of massive hepatic failure in patient, leading to coma, is the abnormal production of methane, ethane and dimethylsulfide.
