A syndrome has been described comprising varicose veins of the lower extremity arising from the internal iliac vein. The condition occurs in multiparous women, arises in pregnancy, and tends to remission after delivery. Subsequent pregnancies cause severe exacerbations and each menstrual period is associated with a mild exacerbation. A history of vulvar or gluteal varices in pregnancy is typical. The long or short saphenous veins may not be involved, but a varicose network extending down the thigh from the gluteal fold or pudendal region is pathognomonic. Therapy is palliative only, consists of the avoidance of future pregnancy, surgical removal of incompetent saphenous trunks and communicators when present, and frequent injections of sclerosing agents.
