Abstrakt: Introduction: The aim of this study was to determine the effectiveness of acceptance, commitment and reality therapy based on death stress and biomarkers of FBS and HbA1c in the elderly with type 2 diabetes. Materials and Methods: The research design in terms of practical purpose in terms of the nature of a quasi-experimental study was pre-test-post-test with a control group and random assignment. The statistical population of the study included all elderly diabetic patients (type 2) referred to specialized clinics of diabetic patients in Shiraz in 2020. Among those referred to these centers, 60 people were selected by purposive sampling method and were replaced in 3 groups of 20 people. Participants were assessed using the Templer Death Anxiety questionnaire (DAS). Then, the participants of group therapy based on acceptance, commitment and reality therapy were trained in 8 sessions of 90 Minutes and the control group did not receive any training. Data were analyzed in SPSS software version 23. In order to test the research hypotheses, multivariate analysis method and t-test were used for two independent groups. Results: The results showed that treatment based on acceptance, commitment and reality therapy had a significant effect on death stress and biomarkers of FBS and HbA1c in the elderly with type 2 diabetes (P <0.05). The results of pairwise comparison of the two groups through Tukey post hoc test showed the difference between the mean stress due to death and FBS and HbA1c biomarkers between the control group and the commitment and acceptance treatment group and the reality therapy group. Conclusion: Therefore, the results can be said that acceptance, commitment and reality therapy can be used to reduce stress caused by death, and these therapies can be used to improve the biomarkers of FBS and HbA1c in the elderly with type 2 diabetes.
