COMMON PATTERNS OF RECURRENCE IN EPITHELIAL OVARIAN CANCERS.

Abstrakt: Background: The primary objective of ovarian cancer debulking is complete resection of all grossly visible disease both in primary debulking surgery (PDS) as well as in interval debulking surgery (IDS), in cases of early and advanced ovarian cancers respectively. Even after achieving the desired objective, most of the patients develop recurrent disease. Objective: The purpose of our study was to analyse the common patterns of recurrence in patients with epithelial ovarian cancers during their follow-up. Materials and methods: After completion of primary therapy, one hundred patients with no residual disease were followed up for one year (2020-2021) and biochemical recurrence was detected using tumour markers human epididymis protein 4 (HE4) and carbohydrate antigen-125 (CA-125), followed by its verification using CECT abdomen/pelvis and various patterns of recurrence were identified. Results: A total of one hundred patients were followed up, out of which 31(31%) patients developed recurrence during the first year of follow up, in 69(69%) patients no recurrence was detected. The features of recurrence noted were: pelvic mass in 25(80.6%), omental/peritoneal metastasis in 24(77.4%), ascites in 19(61.3%), retroperitoneal lymph nodes in 6(19.4%) and pleural effusion in 6(19.4%) patients. The number of recurrences detected was one in 23(74.2%) and two recurrences in 8(25.8%) patients. Conclusion: Most common feature of recurrence in our study was a pelvic mass followed by omental/peritoneal metastasis, ascites, nodal disease and pleural effusion. Recurrence at unusual sites was not detected in any of the patients during the first year of follow-up. [ABSTRACT FROM AUTHOR]
