"It is not unusual for patient to ask about MRI and I discuss the following MRI brain and SC are important at diagnosis as it informs us about the burden of the disease in the brain and SC predilection of the disease (supra or infratentorial), ongoing inflammation, chronicity of the disease (black hole, brain and spinal cord atrophy). Once on a disease modifying therapy, if there is evidence for disease activity on MRI, then we need to have a discussion regarding whether we believe the current treatment is meeting our treatment goals. MRI brain is done more often than SC for disease surveillance as the patient is likely to show new symptoms with a new SC lesion, while this might not be true in the brain (can have several new lesions with subtle or no symptoms).
