Indolent parapharyngeal abscess of undetermined origin: a case report.

Abstrakt: Introduction: Abscess in the parapharyngeal region is mainly of odontology or tonsillar etiology. The symptoms are variable and may include pain, dysphagia and dyspnea. The neoplastic and metastatic hypotheses must be considered in atypical evolutions and in about 20% of the cases, the origin is undetermined. It is believed that a part of them corresponds to undiagnosed congenital deformities. The usual treatment is based on early diagnosis, intravenous antibiotic therapy, maintenance of airway patency and surgical approaches. Objectives: To report a case of parapharyngeal abscess clinically mimicking a malignancy process. Resumed report: Male, 58 years old, no comorbidities, started with a progressive feeling of pharyngeal mass for 4 months. On physical examination, a voluminous tumor was observed in the left parapharyngeal region. A contrast-enhanced Computed Tomography showed a lesion with homogeneous peripheral enhancement which started in the cavum and extended until the hyoid bone. A local needle puncture was performed draining about 32 ml of purulent secretion. On admission, antibiotic therapy was initiated, the clinical investigation including infectious and neoplastic screenings was negative, and the material culture showed Burkholderia cenocepacia. The patient was discharged after 10 days and remained without complications or recurrences. Conclusion: The only positive finding in this case investigation was the growth of Burkholderia cenocepacia, whose immune escape mechanisms of the host still require further studies. It is important to know the differential diagnoses in atypical cases due to the topographical relation between the parapharyngeal space and vital structures. [ABSTRACT FROM AUTHOR]
