Mucosal leishmaniasis: a case report.

Abstrakt: Introduction: Leishmaniasis is a chronic and infectious disease caused by protozoa of the genus Leishmania. Transmission occurs through the bite of sandflies (Phlebotomus or Lutzomya) mosquitoes. It is more frequent in males. Mucosal leishmaniasis is a form of cutaneous leishmaniasis. The main nasal signs and symptoms include nasal obstruction, epistaxis, erythema, infiltration, erosion, ulceration with subsequent granuloma formation in the anterior nasal septum and perforation of the nasal septum. Treatment is performed with antimony - N-methyl Glucamine. Objectives: Report a case of mucosal leishmaniasis with nasal involvement. Resumed report: MAM, 56 years old, female, smoker, history of cocaine use, complaint of epistaxis and pain in the nasal region with 1 year of evolution. On physical examination, edema and hyperemia of the nasal vestibule mucosa, ulcero-crusted lesions and anterior septal perforation were evident. Admitted for diagnostic investigation. Laboratory tests and biopsy were performed, with results increased C3, normal C4 and CH50, nonreactive anticardiolipin antibody IgM, negative serology for Hep B and C, negative syphilis and HIV, negative C-ANCA and P-ANCA, negative ANA and lupus anticoagulant. Anatomopathological and immunohistochemical: positive BCL2, CD3 and CD20 markers, cell proliferation antigen (Ki67) 5%. Compatible with Leishmaniasis. The treatment was performed with antimony - N-methyl Glucamine. Patient remains under follow-up. Conclusion: Early diagnosis and treatment of Leishmaniasis are important in order to prevent deformities and functional impairment. It is important to consider leishmaniasis as a differential diagnosis of ulcerocrustal nasal lesions. [ABSTRACT FROM AUTHOR]
