Nasopharyngeal squamous cell carcinoma: case report in a Pediatric Hospital.

Abstrakt: Introduction: Nasopharyngeal carcinoma is rare (0.25% of tumors) and presents with nonspecific symptoms, such as tinnitus, hearing loss, facial pain and nasal obstruction, with most patients diagnosed only in advanced stages. 20% of cases occur in people under 30 years of age. Non-keratinized tumors are related to EBV. Objective: To report a case of nasopharyngeal carcinoma in pediatrics. Resumed report: BTG, 8 years old, female, complaining of pain in the mandibular and left periauricular region, hearing loss and otorrhea with 6 months of evolution. Evolved with trismus. Evaluated at another service and suspected of having a retropharyngeal abscess, drainage was attempted and a stony lesion on the posterior wall of the rhinopharynx was evidenced intraoperatively. Transferred to this service for investigation. Audiometry with moderate left conductive loss. On resonance, a solid lesion with an area of necrosis in the left parapharyngeal space, occupying the infratemporal fossa, naso and oropharynx, left prevertebral space and Eustachian tube, with diffusion restriction, encompassing the extracranial segment of the left internal carotid artery, measuring 7.0x4,2x5,2cm. Left jugulocarotid adenomegaly suggestive of metastasis. A biopsy of the rhinopharyngeal lesion was performed, resulting in a compatible non-keratinizing undifferentiated nasopharyngeal squamous cell carcinoma EIII (T3N1M0). Patient undergoing chemoradiotherapy. It evolved with a reduction of the lesion to 1.2x1.9x0.8cm, which may correspond to a scar lesion. Conclusion: Due to the nonspecific nature of nasopharyngeal carcinoma, there is a need for high diagnostic suspicion and attention to performing nasofibroscopy for early diagnosis.
