Introduction: Subglottic stenosis can be defined as the narrowing of the airways below the vocal folds to the lower border of the cricoid cartilage. May be congenital or acquired, with traumatic intubation being the most common cause. Objectives: Evidence of subglottic stenosis as a complication of prolonged OTI and demonstrate its resolution through a surgical approach. Resumed report: Z.P.M.C.D, female, 79 years old with hypertension, diabetes, hypothyroidism presented dyspnea and snoring in 2020 with progressive worsening evolving to severe acute respiratory failure requiring orotracheal intubation and tracheostomy in October 2021. After 4 months, bronchoscopy was performed under sedation, visualizing swollen larynx and >50% obstruction in the subglottic region with patent trachea. In April 2022, she underwent nasofibrolaryngotracheoscopy with edema involving the lower portion of the right vocal fold (preserved mobility) and subtotal stenosis involving 90% of the subglottic lumen, with an irregular appearance extending to the supratracheostoma region and cervical CT with reduction of the subglottic lumen by important stenosis of the region. A new tracheostomy and laryngeal microsurgery were performed using a CO2 Ultra pulse Duo laser on April 22, 2022 and a new surgical procedure after 20 days with a Diode laser. Post-surgical control nasofibrolaryngotracheoscopy was performed weekly, showing good evolution and patent lumen. Conclusion: The inflammatory process due to trauma and prolonged OTI time may contribute to the emergence of laryngotracheal strictures. In this case, the stenosis was resolved through laser microsurgery, however, care and reduction of intubation time should be prioritized to avoid complications. [ABSTRACT FROM AUTHOR] |
