A case of tachycardia and lethargy immediately after intravesical lidocaine instillation: what can we learn?

Abstrakt: A 63-year-old woman with interstitial cystitis experienced lethargy, confusion, and tachycardia following intravesical lidocaine insertion. Her symptoms resolved with bladder drainage and clinical monitoring. We performed a literature review assessing the systemic adverse events associated with intravesical lidocaine. In our review, five patients receiving intravesical lidocaine experienced systemic adverse events; four of whom had transient ischemic attack symptoms. The authors believed the transient ischemic attacks were likely an adverse effect of epinephrine. This finding raised questions surrounding the combined administration of epinephrine and lidocaine, a pairing that may not be required given the associated potential risks. Other side effects were related to lidocaine doses that exceeded the safe concentrations established through intravenous toxicity studies. Future pharmacological studies should identify safe doses of intravesical lidocaine and risk factors for systemic absorption, especially in pathologic bladder states, such as infection, bladder augmentation, and procedure type.
