Neuromodulation is effective in a proportion of patients with demonstrated storage phase abnormalities, specifically detrusor overactivity,7 but chronic nonobstructive urinary retention is a not defined, not quantified, or quantify-able word combination, currently used as being a "diagnosis." It is possible to quantify and exclude bladder outflow obstruction when a patient voids ineffectively (low voiding percentage or postvoid residual [PVR]) but these data are never reported in studies reporting neuromodulation. Much better diagnosis and diagnostic categories for (representative) inability to void completely or "incomplete voiding" are needed for many reasons, apart from solid and quantifiable definitions of bladder outflow obstruction and detrusor voiding contraction.
