Abstrakt:	Sir, Tolosa-Hunt syndrome (THS) is a rare disease characterized by unilateral painful ophthalmoplegia of unknown etiology. Hence, infliximab therapy can be considered as the treatment of choice in resistant cases of THS. Other causes of painful ophthalmoplegia were excluded by blood investigations, lumbar puncture, and neuroimaging, thereby meeting the International Headache Society (IHS) criteria for diagnosis of THS.[[2]],[[3]] THS usually responds well to steroid therapy.
