Background: Acute pancreatitis is as a critical ailment where cases present with pain in the abdomen and is associated with increased levels of enzymes secreted by pancreas in serum. In present study, we aimed to assess the accuracy of PANC 3 scoring system in predicting severity in an attack of acute pancreatitis, as well as to evaluate the predictability of PANC 3 scoring system in correlation with RANSON'S scoring system. Material and Methods: Present study was single-center, prospective, observational study, conducted in subjects > 18 years & duration of onset < 48 hours, diagnosis of acute pancreatitis was confirmed. Severe Acute Pancreatitis was predicted using the Ranson's score & PANC 3 score. Results: Out of the total 98 cases, 16.33% were females and 83.67% were males. 67% cases were mild AP, 11 cases developed pleural effusion which is 84.6% cases having SAP. PANC3 score predicted SAP if all three criteria's were positive, and out of the 13 cases of SAP, 4 (30%) had all three positive criteria's, 3(23.07%) had two positive criteria, and 3 (23.07%) had only one positive criteria and 23.07% had no positive criteria. Out of the 13 cases of SAP, 6 died. PANC3 score was found to have 30.77% sensitivity, 89.8% specificity. The 80% PPV, 90.32% NPV in predicting SAP. 89.8% was the accuracy of diagnosing SAP. But on plotting the receiver operating characteristic curve, are under the curve was 0.648 indicating it to be a poor predictor of SAP. Conclusion: PANC3 scoring system has high predictability, accuracy and specificity, but low sensitivity therefore it is a good predictor of SAP with poor accuracy. [ABSTRACT FROM AUTHOR] |
