The diagnosis of periappendicitis was made after finding an inflammatory neutrophilic infiltrate in the serosa of the appendix with sparing of the appendiceal mucosa and submucosa (Fig. Periappendicitis is a pathological diagnosis defined as inflammation of the serosa of the appendix that may extend into the muscularis propria but spares the mucosa.1 Whilst this pathology may be present in any patient with widespread peritonitis, there is uncertainty about what this finding means after appendicectomy for suspected acute appendicitis I when no other intra-abdominal pathology has been identified at operation i . Second, all four of our cases were considered to have acute appendicitis clinically and at operation; the diagnosis of periappendicitis was made post-operatively only after examining sections from the entire appendix to exclude focal appendicitis.
