| Abstrakt: | Likewise, it represents an encouraging movement away from an over-reliance on methods such as root cause analysis (RCA), which are often seen as promoting a reductionist and over-simplified explanation of why patient safety incidents happen. One challenge for the future is that methods such as RCA have been around within healthcare for several decades and it will be difficult to replace them with systems-based HFE methods.
