Blue (or purple) toes: chilblains or chilblain lupus-like lesions are a manifestation of Aicardi-Goutières syndrome and familial chilblain lupus. The differential diagnosis for chilblain-like lesions in children and adolescents includes AGS, familial chilblain lesions, STING-associated vasculopathy with onset in infancy (SAVI), and chilblain-like lesions associated with COVID-19. Particularly, both AGS and familial chilblain lupus show interferon (IFN)-induced microvascular injury which may be the cause of their clinical and histopathological similarities.1 Within the context of the COVID-19 pandemic, chilblain-like acral lesions have been reported in young patients with no or mild symptoms compatible with COVID-19.
