A smiie is a symbol of beauty and weiibeing in our modern society, human facial expression transcending language, culture, race, gender, time, and socioeconomic differences. An esthetic smiie consists of three main components: the teeth, the iip framework, and the gingival scaffold. In some patients, the altered relationship between the teeth, the aiveoiar bone, and the soft tissue may result in the ciinicai condition known as gummy smiie. A single factor or a combination of factors may be present in patients with this ciinicai condition, including altered passive eruption (APE), verticai maxiiiary excess, and a short or hyperactive upper iip. The present articie reports on a 31-year-oid femaie patient who presented for a consuitation. The patient, who was in good generai heaith with no significant medicai history, was dissatisfied with her tooth esthetics. She had symmetric faciai features with a iong face appearance, retruded chin, protruded maxiiiary incisors, and excessive gingivai dispiay that indicated evident APE. In particuiar, the present case report aims to describe a muitidiscipiinary treatment invoiving a phase of orthodontics associated with maxiiiofaciai surgery, and the periodontai surgicai sequence of esthetic crown iengthening for the treatment of APE.
