Relevance: Due to the fact that secondary prevention and dispensary treatment have been almost reduced in Ukraine during the last years a set of innovative medical and economic standards is suggested for their renewal and improvement; their implementation should be tested firstly by means of a field experiment. Objective: To prove the effectiveness of scientifically based organizational and economic standards of criteria and regulations in the process of improving the health of the population in the area of responsibility of the family physician using diagnostic and treatment management within the "Pilot Project" in the three levels of health care areas of the district. Material and Methods: Since the algorithm of the complex of medical and economic standards for secondary prevention and dispensary treatment were developed on the basis of one service - otorhinolaryngology, and the positive effect of health improvement should be achieved for all medical specialties during the preparatory phase, using the goal prognosis method, a team of medical professionals will retrospectively analyze a significant number of histories of various diseases at the tertiary level in the healthcare institution to differentiate these diseases into clinical-statistical groups, diagnosis related groups and their degree of complexity, as it is already done for ENT diseases, which will enable using appropriate software and recommended formulae to calculate quantitative and qualitative indicators of the amount of medical work to determine the level of work load, its cost, and personal attainment coefficient of each physician. Results: Use of our standards, criteria and guidelines in the process of pilot project makes it possible of the basis of timing to determine the standard and over normative volumes of medical work, indicators of which by certain calculations are transformed into a mechanism lor determining the working load of the doctors, Its cost, which allows differentiate deciding the level of their material incentives, i.e. those technological divisions, which are essential to create a system of our national insurance medicine.
