Anaplastic cancer is a rare form of thyroid cancer that is associated with a poor prognosis. Combined mediastinal cancer of the thyroid gland is extremely rare. Surgical treatment followed by radiation therapy is the main approach in these cases. This manuscript presents a clinical case of a 49-yearold woman in whom prophylactic ultrasound of the neck and subsequent chest radiography and CT revealed a mediastinal mass arising from the left lobe of the thyroid gland. Total thyroidectomy, thymectomy, resection of the left jugular vein and left brachiocephalic vein were performed. The management regarding diagnosis and surgical treatment is discussed.
