ВЪЗПРИЕМАНЕТО ОТ СТРАНА НА ЛИЧНИТЕ ЛЕКАРИ НА ТРУДНОСТИТЕ ПРИ УПРАВЛЕНИЕТО НА ПОЛИМОРБИДНИ ПАЦИЕНТИ В ЗАВИСИМОСТ ОТ ПСИХОЛОГИЧНИЯ КОНСТРУКТ ЛОКУС НА КОНТРОЛ.

Abstrakt: Multimorbidity is a growing problem and poses a major challenge to health care systems around the world. GPs and primary care teams play a key role in the management of multimorbidity patients. This requires changes in the way health systems function to optimize care for this group of patients. Along with the pointed-out factors, the physician's personality also influences the overall service process and the doctor--patient relationship. It is generally accepted that to some extent the locus of control could determine the kind of behavior and the way we approach a situation. The aim of the study was to examine the attitudes of GPs to the difficulties in managing multimorbidity patients depending on their locus of control. A representative survey was conducted among 319 randomly selected GPs. An original questionnaire was developed based on a literature review on the problem. The test for the localization of control, adapted and validated for the Bulgarian population, was applied to assess the locus of control. The results have shown that the management of multimorbidity patients is associated with overcoming various difficulties. GPs with an external locus of control have a statistically significantly higher score in the following domains: lack of human resources; poor collaboration with social services, difficulties in assessing polypharmacy; reporting of adverse drug events; poor communication with patients and relatives/caregivers; challenges in shared decision-making and poor patient cooperation. In conclusion, the study has shown an association between an established psychological construct "locus of control" and identified difficulties in managing multimorbidity patients. GPs with an external locus of control have given a statistically significantly higher score for most difficulties. For them, multimorbidity patients have proved to be even more challenging.
