(Sir William Osler) Gestational diabetes mellitus (GDM) is becoming increasingly common and carries risks to pregnancy and long-term maternal and offspring health. In a large prospective multicentre cohort study, van Hoorn and colleagues report that a GDM screening strategy using validated first trimester prediction models offers a higher net clinical benefit compared with single risk factor-based approaches (van Hoorn et al. I BJOG i 2021;128:645-54). Prediction models may then make Osler's art of probability more useful for the individual, the clinical team and the health system to optimise personalised care and improve health outcomes.
