B Author Information b An event is serious (based on the ICH definition) when the patient outcome is: * death * life-threatening * hospitalisation * disability * congenital anomaly * other medically important event A 52-year-old woman exhibited a lack of efficacy with botulinum-toxin, unspecified steroids, immune-globulin, mycophenolate, nitroglycerin and sildenafil, while being treated for skin ulceration [1]. The woman, who had a history of melanoma differentiation associated protein 5 (MDA5) amyopathic dermatomyositis and connective tissue disease-associated interstitial lung disease (CTD-ILD), was admitted with severe pain in bilateral hands along with findings concerning for digital ischaemia and worsening cutaneous skin ulcerations. During hospitalisation, her skin ulcerations (negative arterial dopplers) deteriorated continuously despite treatment with unspecified steroids, mycophenolate, sildenafil, topical nitroglycerin, botulinum toxin injections and IV immune-globulin (lack of efficacy) [ I dosages not stated; not all routes stated i ].
