West Nile fever (WNF) is a vector-borne viral infectious disease. West Nile virus belongs to Flaviviridae family. Birds are the natural reservoir and source of infection and mosquitoes - the vector of the infection. The number of WNF cases in Europe and Bulgaria has increased over the last few years. Aim: To present the clinical presentation of WNF in Bulgarian patients during 2018 and 2019. Materials and Methods: 23 patients with WNF. The methods used include epidemiological survey, clinical observation, clinical- laboratory, microbiological and serological tests, molecular-biology techniques (polymerase chain reaction - PCR), medical imaging - computed tomography (CT) and magnetic resonance imaging (MRI). Lumbar puncture was performed in 15 of the patients. Results: 15 (65.2%) of the patients were treated in 2018 and 8 (34.8%) - in 2019. The most common symptoms included fever and adynamia (87%), headache and gastrointestinal disorders (52%). 12 of the patients (52.2%) presented with quantitative and 7 (30.4%) with qualitative consciousness disorders. Nuchal rigidity was found in 12 patients (52.2%), brisk tendon reflexes in 11 (47.8%) and positive Romberg test in 12 (52.2%) of the patients. 13 of the WNF cases (56.5%) were confirmed and 10 (43.5%) - probable. The most commonly observed complications were pneumonia, pulmonary stasis, pancreatitis and neurological disorders. Conclusion: The number of WNF cases in Bulgaria has increased. Young people in active age are predominantly affected. Most of the cases were registered during the summer when mosquitoes are active. Detailed anamnesis and epidemiological anamnesis, opportune specimens' collection and careful result interpretation contribute to establish the diagnosis. [ABSTRACT FROM AUTHOR] |
