Abstrakt: We describe a case of the malignant middle cerebral artery syndrome due to a left middle cerebral artery infarct developing in right-handed 29-year old male with a history of polysubstance abuse. Timely surgical intervention by decompressive hemicraniectomy led to survival and subsequent successful rehabilitation with the ability to perform activities of daily living independently. The literature on survival considerations of a middle cerebral artery associated stroke is reviewed. Doctors involved in stroke care need to be aware of the various surgical interventions available for stroke care in the Eastern Caribbean. [ABSTRACT FROM AUTHOR]
