Abstrakt: | The article discusses a study which evaluated the impact on low-income diabetes patients of a diabetes education and management services provided by a clinical team from Heart of Ohio Family Health in Columbus, Ohio. Topics covered include a change in A1C scores among patients seen by the pharmacy team for diabetes management and evidence that pharmacist diabetes services are capable of significantly lowering A1C levels of patients who receive care in Federally Qualified Health Centers. |
