Misoprostol is synthetic prostaglandin indicated for the management of gastro duodenalulcers and used the management of the first trimester missed abortion and it can be used in different regimes and routes. To compare the efficacy of Misoprostol in first trimester missed abortion management through two sublingual and vaginal routes of administration. Prospective clinical trial conducted among 100 women who had missed abort ion confirmed by ultra sonography, they divided randomly into two equal groups. The 1st group received 600 microg of sublingual misoprostol and the other group received 800 microg vaginal misoprosto 1.2nd dose was given for those who failed to start abortion after 48 hours. After two weeks of follow up and when uterus was empty on USG and the women didn’t have bleeding we considered the medical treatment was successful and no need for surgical intervention. The treatment success rate was high in the vaginal route with statistical significance (sublingual 66%, vaginal 84% P = 0.023,95%CI). Also, women in the sublingual group had experienced more complications, but the difference was not that significant (p value=0.206). 90%of the vaginal group and 61% of the sublingual group started medical abortion after taking the 1st dose. More than half of the women with vaginal group versus one third of sublingual groupstarted expulsion of consumption products in the 1st 24 hours(p value=0.012). The higher success rate (85%) of complete medical abortion was in patients with gestational ages less than ten weeks (p value=0.000). Medical management of missed abortion by the misoprostol can be done safely at home unless there is some indications for the admission. The Vaginal group had higher success rate, less side effects and bleeding, shorter induction abortion interval (IAI), than sublingual group and the association was significant.
