Background: Ulcerative colitis is an immune-mediated disease characterized by mucosal inflammation that generally begins in the rectum and involves a variable extent of proximal colon. The aim of our study was to evaluate the clinical and phenotypic characteristics of paediatric ulcerative colitis. Material and methods: A single center observational retrospective study, including 50 children (27 girls and 23 boys) with ulcerative colitis treated in the Department of Gastroenterology and Hepatology, at University Pediatric Hospital “Prof. Ivan Mitev”, Sofia between March 2011 and February 2018). Results: Bloody diarrhea was the debuting symptom in most of our patients 23/50 (46%). Other presenting features were non-bloody diarrhea 17/50 (34%) and abdominal pain 6/50 (12%). In 4 cases (8%) the disease presented with an extraintestinal manifestation. At the time of diagnosis more than half of our patients 29/50 (58%) had extended disease (extensive or pancolitis). According to the Paris classification, 22/50 (44%) presented with pancolitis (E4), 7/50 (14%) with extensive colitis (E3), 15/50 (30%) with left-sided colitis (E2) and 6/50 (12%) with ulcerative proctitis (E1). Severe disease (PUCAI>65) was observed in 18/50 (36%) of our patients. Conclusion: Pediatric-onset ulcerative colitis is characterized by extensive bowel involvement and severe clinical presentation.
