The authors comment on the findings of a single-site randomized clinical trial of a tailored 3-month intervention for patients with chronic obstructive pulmonary disease (COPD) that integrated transitional care support and chronic disease self-management. They discuss the unique characteristics of the study that may have contributed to the success of the intervention. They also describe the benefits of using prediction models to identify patients at risk for COPD readmission. |
