The article discusses the use of a cheat sheet to identify International Classification of Diseases (ICD)-10 codes that correspond to Hierarchical Condition Categories (HCC). Topics covered include a simple workflow intervention and tool for optimizing payers' risk adjustment calculations, and an example of its application. Also noted is the need for clinics in value-based payment settings to document and report comprehensive qualifying diagnoses for each patient annually. |
