Elective caesarean section versus defensive caesarean section.

Abstrakt: Objectives. Our main objective was to identify the exact causes of the ascending percentage of the caesarean sections in Romania, and to create a database and a measurement plan in order to quantify the amount of caesarean delivery on maternal request. Defensive caesarean section (CS) is one of the most common manifestations of medicine in Romanian obstetrics. We performed a prospective, observational, qualitative, multicentric study to quantify the impact of this type of practice which is used by obstetricians to avoid medico-legal complaints and decrease the frequency of malpractice litigations. Methods. We analyzed the percentages of caesarean sections undertaken in five tertiary departments of obstetrics and gynecology, and we also interviewed 73 obstetricians from three of the departments, to assess their opinion regarding defensive CS and CS delivery performed upon maternal request. Results. All studies showed a significant increase in the number of caesarean delivery on maternal requests undertaken in Romania, even though they have not yet been legalized in Romania. After interviewing 71 respondents, 51 (69.9%) stated that they practice defensive CS; 63 (86.3%) declared that their choice of birth delivery is influenced by the risk of being accused of malpractice; 60 (82.2%) considered that it is normal for the patient to be able to choose how to give birth; 57 (78.1%) were in favor of legalizing CS delivery upon maternal request, and 63 (86.3%) declared that they consulted their patients on their delivery preferences. Conclusions. In 2014, Romania has reached a record of caesarean sections, occupying the second place after Turkey (in Europe), with Bucharest recording 740.8 caesarean sections out of one thousand babies to be born. The results also indicate that defensive caesarean section is a widespread practice among practitioners of obstetrics in Romania. [ABSTRACT FROM AUTHOR]
